What if there is a problem or you wish to make a complaint?

If you are worried, upset or anxious as a result of answering any of the survey questions, or about anything else in your life, getting advice or talking to someone can help. We have compiled an information sheet called ‘Where to Go for Information and Advice’, which includes details about a range of places where support can be obtained. The interviewer will leave a copy of this leaflet with you at each face-to-face interview and the information can also be found on this page.

If you remain unhappy and wish to complain formally, you can do this by contacting your local PALS (Patient Advice and Liaison Service) team. You can find officers from PALS in your local hospital, or your nearest PALS office on the NHS Choices website (www.nhs.uk), by asking your GP surgery, or by phoning NHS 111.
